REGISTRATION BY MAIL

1	Please open and print CITA Registration Form. This Registration Form is in Adobe Portable Document Format (PDF). If you do not have a copy of Adobe Acrobat Reader software installed on your computer, please download and install it for free from Adobe website.	REGISTRATION FORM
2	Please create an account for yourself on CITA Online webpage. Please remember your unique user name and password and maintain them safeguard at any time. You will need to indicate your user name in the Registration Form.	CREATE AN ACCOUNT
3	Please complete and sign the Registration Form.
4	Do not forget to indicate your amount and method of payment. If your payment method is credit card, please provide information on credit card type and details as well as sign credit card portion of the Registration Form. If your payment method is Certified Cheque or Bank Draft, please make it payable to Canadian Immigration Training Academy and indicate your full name and course number on it.
